You do not need formal language. Tell us what happened, when it happened, who was involved and what you would like to see happen next. Share only the information you are comfortable providing in this first message.
Participants have the right to expect safe, respectful and competent support. Raising a concern helps us protect that standard.
Independent options
You may contact the NDIS Quality and Safeguards Commission to report an issue or make a complaint about an NDIS provider or worker. You can use its online reporting pathway or call 1800 035 544.
