A thoughtful first conversation helps us understand whether we may be the right provider. Participants, family members, nominees, support coordinators and referral professionals are all welcome to contact us.
You do not need to have every answer. Share what you know, and we will explain the next step clearly.
Participant enquiries
Help us get to know you.
The questions in this form help us understand the participant’s priorities before we speak. We want to know what is important to the person, what support is needed now and what progress they would like to make.
Do not include an NDIS number, medical records or sensitive documents in this initial enquiry. We can explain a safe way to share relevant information later if needed.
Support coordinators & referral professionals
A clear, participant-led referral response.
We welcome enquiries from support coordinators, plan managers, allied health professionals, hospitals, community organisations and participant representatives.
Share the participant’s goals, suburb, preferred schedule, support requirements, funding arrangement and ideal start timeframe. We will review the request against current capacity and explain whether we can progress to a conversation with the participant.
